How to get COVID Smart Vaccination Certificate Online in Sri Lanka

People who want to travel abroad now can get Smart Vaccine Certificate officially from WHO Sri lanka.

This programme is a collaboration with the Ministry of Health – Sri Lanka and WHO Sri Lanka. They have made an online portal to register for the Smart Vaccine Certificate.

Currently these features is available for the people who travel to abroad. But in upcoming days, government will decide to provide this certificate for everyone.

2. Full Name as in Passport
3. Permanent Address
4. District of Residence
5. National Identity Card Number
6. Passport Number
7. Mobile Phone Number
8. Email Address
9. Date of Departure
10. Upload the following required images
    • Photo of Passport
      (detail Page)
    • Photo of NIC (Front Page)
    • Photo of NIC (Back Page)
    • Photo of Vaccination Card (Front Page)
    • Photo of Vaccination Card (Back Page)
    • Document to prove the date of traveling
11. Special Notes (If there is any)
